Things You Should Know…

- PowerStix Phlebotomy Co LLC does not collect specimens without a provider’s written order. These orders can be faxed to 999-99-9999 or emailed to admin@PowerStixPhlebotomy.com

 

- If you don't have provider's order we can request an order to one of our providers.

 

- We handle the specimen from the time of collection until its delivery to the lab or shipping facility. PowerStix Phlebotomy Co, LLC is not responsible for any errors that may occur once the specimen is delivered to the lab or shipping facility.

 

- Please know that PowerStix Phlebotomy Co, LLC does not bill insurance. We will take your insurance information for the lab and the lab will bill for the tests collected.

 

- Patients should be aware that insurance does not cover the cost for the phlebotomist to collect and transport the specimen. PowerStix Phlebotomy Co, LLC will expect payment at the time of service.

 

- Cost depends on the services and the location of where the service will be provided. You will receive an invoice for your scheduled service prior to the appointment. Payment will be due at the time the service is provided.
